Objectives
Participants will apply a hybrid of clinical/medical and social experiential
evidence-based practices that connect and communicate coordination through intra-
and inter-organizational collaboration without diluting the key elements of either
paradigm. (understanding hybrid model)
Participants will promote the interoperability of two models including the
access to, and sharing of, medical and social history information from different
systemsto improve resident care and outcomes. (improve resident experience)
Participants will recognize the significant advantages of having more
complete and accurate resident information and data for making better informed
decisions regarding resident treatment and prosocial networking needs. (enhance
decision-making)
Participants will facilitate better service provider collaboration and improve
program efficiency. (streamline operations)
Participants will apply the various levels of interoperability, i.e.,
foundational, structural, semantic and organizational, allowing for information sharing
and service integration that enhances the effectiveness of each model. (levels of
interoperability support recovery eco-system)
